At Freedom Care we adopt a person centered care approach with all of our service users. Person centered care sees the patient as an equal partner in the planning, developing and assessing of care to make sure it’s the most appropriate for their specialist care needs. Key to this process is putting the patient and their families at the heart of all decisions.

The main aim of a person centered care approach is to be primarily user focused. Based on a strong team philosophy we encourage independence and autonomy as well as provide choice and control. Every user’s specialist care needs are taken into account with a bespoke programme designed for each patient.

The main elements of person centered care is:

  • Respect, compassion and dignity – these are the essential foundations for the greater involvement of people in their own care.
  • Shared decision making – this requires the involvement of patients as equal partners in their specialist care. Tools to support shared decision making can include self-management support, access to personal health records, personal health budgets, care planning and shared treatment decisions.
  • Collective patient and public involvement – this is about involving people in decisions about the design and delivery of services.

Through a person centered care approach, our specialist care services teams help to teach every one of our service users valuable life skills; helping them make informed decisions and lifestyle choices.
